Job summary

Nest Health in Arizona is seeking a Nurse Practitioner to deliver whole-family primary, acute, and pediatric care in mobile, community settings or homes. You will work with an interdisciplinary team to provide evidence-based care and coordinate services for high-risk families.

The role involves managing a panel of patients, performing assessments, ordering tests, prescribing medications, and educating families. Arizona NP license and strong commitment to underserved populations are required.

Qualifications

• Completion of Nurse Practitioner accredited program and an active, unrestricted license in Arizona.
• At least 2 years' experience providing clinical services to pediatric and adult patients with chronic conditions.
• Proven skills in blood drawing, patient assessments and care plan reinforcement.
• Independent clinical decision-making and problem-solving abilities.
• Available to work full-time, four days/ten hours per week with rotating evenings and weekends.
• Maintains CPR certification (BLS).
• Experience leading an interdisciplinary care team is preferred.
• Startup experience is a plus. Access to reliable transportation required.

Responsibilities

• Manage a panel of families as the primary care provider to improve whole-family health.
• Practice with an interdisciplinary team including MDs, LCSWs, RNs, psychiatrists, and medical assistants.
• Utilize Nest Health’s EHR to document interactions, track tasks, and communicate with team and families.
• Attend daily team huddles and weekly case conferences to set health goals.
• Propose treatments, prescribe medications, and educate patients about conditions.
• Order, administer, obtain, and record diagnostic tests.
• Provide advocacy and navigation support for health and social services.
• Participate in outreach to Nest families to explain program details.
