Nest Health in Arizona seeks a Nurse Practitioner to provide whole-family primary, acute, chronic, and pediatric care across mobile, community facilities, and home visits as needed. You will work with an interdisciplinary team to deliver evidence-based care and education.

The role is hybrid, involving travel to rural communities and virtual care, with a rotating on-call schedule. You’ll educate families, coordinate care, and support social services navigation to improve outcomes.

Qualifikationen

• Completion of NP program and active Arizona license.
• Experience delivering care to pediatric and adult patients.
• Ability to work autonomously within an interdisciplinary team.

Aufgaben

• Provide whole-family primary, acute, chronic, and pediatric care.
• Collaborate with physicians, behavioral health, dieticians, and medical assistants.
• Use EHR to document interactions and coordinate care.
• Attend daily huddles and weekly case conferences to set health goals.
• Order and interpret diagnostic tests and educate patients.
