Job summary

EasyHealth is seeking Pediatric Nurse Practitioners to deliver in-home preventive care in Flagstaff, Arizona. You will meet members where they are and perform comprehensive assessments to identify care gaps and coordinate ongoing services, outside traditional clinic settings.

Ideal candidates hold an active Arizona NP license, are nationally board certified as PNP-PC, and are Medicaid-registered, with the ability to travel locally for scheduled visits and work autonomously within a supportive

Qualifications

• Must hold an active Arizona Nurse Practitioner license.
• National board certification as a Nurse Practitioner.
• Must be certified Pediatric Nurse Practitioner (PNP-PC).
• Must be Medicaid-registered.
• Comfortable traveling locally for scheduled in-home visits.
• Strong assessment and clinical decision-making skills.
• Able to work independently within a supportive clinical organization.
