Zusammenfassung

Pinnacle Family Services is seeking a Family Centered Treatment (FCT) Specialist to deliver evidence-based services in homes and communities. You will guide families through the FCT phases, develop Service Plans, and coordinate with medical and school partners to ensure holistic care.

FCT Certification is required within one year of hire. The role offers a community-based, flexible schedule with 24/7 on-call crisis support and a strong emphasis on trauma-informed, client-centered care.

Qualifikationen

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• Master's in human services field + 1 year experience.
• Bachelor's in human services field + 2 years experience.
• Bachelor's in non-human services field + 4 years experience.
• Current NC license (including provisional) or board-issued certification in a human services profession.
• Experience must be full-time and supervised, serving children and adolescents with a primary mental health diagnosis, and may be accumulated before or after degree completion.
• Must have a valid driver's license and access to a fully insured, private form of transportation.

Aufgaben

• Deliver Family Centered Treatment services in home and community settings, meeting families where they are.
• Guide children and families through the four phases of the FCT model, set goals and build skills.
• Coordinate with medical, behavioral health, school, and community partners for seamless care.
• Develop and refine individualized Service Plans with families.
• Connect families to community resources, referrals, and aftercare supports.
• Maintain timely documentation in the Electronic Health Record reflecting quality of care.
• Participate in Quality Assurance activities to strengthen programs.
