Pinnacle Family Services, a member of Health Connect America, is seeking a Family Centered Treatment (FCT) Specialist to deliver evidence-based services in homes and communities across North Carolina.

The role requires guiding families through FCT phases, coordinating with medical, behavioral health, school, and community partners, and developing individualized Service Plans. FCT Certification must be completed within one year of hire, with strong professional growth opportunities.

Formación

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• Certification in Family Centered Treatment is required within one year of hire.
• Full-time, supervised experience with children and adolescents with primary mental health needs.

Responsabilidades

• Deliver Family Centered Treatment services primarily in home and community settings.
• Guide families through the four phases of the FCT model and set goals.
• Serve as central coordination point for the family's Treatment Team with partners.
• Develop and implement individualized Service Plans with families.
• Connect families to community resources, referrals, and aftercare supports.
• Maintain timely, accurate documentation in the Electronic Health Record.
• Participate in Quality Assurance activities to strengthen programs.
