Pinnacle Family Services in North Carolina seeks a Family Centered Treatment (FCT) Specialist to deliver evidence-based services in the homes and communities where families live and heal. The role is hands-on, relationship-driven, and focused on real-world outcomes.

As part of Health Connect America’s family of brands, you will work with children, adolescents, and families, coordinating with medical, behavioral health, school, and community partners, developing Service Plans, and documenting in

Competenze

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• One of: Master's in human services field + 1 year experience; Bachelor's in human services field + 2 years; Bachelor's in non-human services field + 4 years.
• Current NC license or board-issued certification in a human services profession.
• Experience must be full-time and supervised, serving children/adolescents with a primary mental health diagnosis.

Mansioni

• Deliver Family Centered Treatment services primarily in home and community settings, meeting families where they are.
• Guide families through the four phases of the FCT model, helping set goals and build skills.
• Coordinate with medical, behavioral health, school, and community partners for whole-person care.
• Develop, implement, and refine individualized Service Plans with families.
• Connect families to community resources, referrals, and aftercare supports beyond treatment.
• Maintain timely documentation in the EHR reflecting quality of care.
• Participate in Quality Assurance activities to strengthen programs.
