Health Connect America seeks dedicated clinicians to provide Family Centered Treatment (FCT) to children and families in home and community settings. You will deliver services, maintain clinical records, develop Service Plans, and collaborate with Treatment Teams to ensure effective, family-centered care.

Qualifikationen

• Bachelor's Degree in human services or related field.
• Two years’ experience working with youth or families.
• FCT Certification preferred.

Aufgaben

• Provide Family Centered Treatment (FCT) services in home/community with a family focus.
• Deliver FCT services to children, adolescents and families with fidelity requirements.
• Develop and maintain Service Plans with Treatment Team involvement.
• Coordinate discharge planning and link to community resources.
• Complete required clinical records and documentation in the EHR.
