Zusammenfassung

Georgia HOPE, part of Health Connect America, is seeking a Family Centered Treatment (FCT) Therapist to provide intensive in-home clinical services to children, adolescents, and families.

You will work in clients' homes and communities with flexible hours, collaborating with treatment teams, schools, and other supports. Regular caseloads include on-call availability, and FCT certification is required within the first year.

Qualifikationen

• Master's Degree required or equivalent in Social Work, Counseling, or related field.
• Two years’ experience working with youth or families.
• Current Family Centered Treatment Certification preferred.

Aufgaben

• Provide Family Centered Treatment (FCT) services in alignment with model fidelity requirements and clinical service definitions.
• Deliver in-home and community-based therapy to children, adolescents, and families using a structured, phase-based treatment model.
• Develop, implement, and adjust service plans in collaboration with treatment teams, families, and other involved systems.
• Facilitate family engagement, goal setting, psychoeducation, and skill-building aligned with identified treatment needs.
• Coordinate care with schools, medical providers, child welfare, juvenile justice, and natural supports to ensure integrated service delivery.
• Maintain accurate, timely documentation, complete all required FCT forms, and support discharge planning and aftercare coordination.
