Job summary

Qualifications

• Bachelor's degree in Social Work or related field.
• Ability to provide in-home services to children and families.
• Experience working with complex and multi-need families.

Responsibilities

• Complete face-to-face intake with family within 24 hours to assess safety and services.
• Develop and maintain a Safety Plan for the family and share with them.
• Act as the point of contact for the family and DCBS throughout the intervention.
• Assist with data gathering and other tasks for family assessment and planning.
• Ensure DCBS receives copies of documentation within contractual timeframes.
• Collaborate with the team to develop Family Agreements and Action Plans.
