Job summary
Maximus is seeking Enrollment Case Managers to support the PA IEB program across Philadelphia County, with daily travel by personal vehicle. This role involves in-home intake visits, assisting applicants to complete documents, and connecting them to waiver program resources to enable timely enrollment.
You will assess eligibility, coordinate with providers and county offices, and maintain accurate records using Excel, Word, and Teams.
Qualifications
- High school diploma or GED with 4+ years of relevant experience or equivalent.
- Residency in Philadelphia County, PA is required.
- Experience in case management, social services, or related field preferred.
Responsibilities
- Travel to applicants' residences for intake visits and eligibility support.
- Conduct in-home assessments and facilitate eligibility determinations.
- Educate applicants about program options and rights/responsibilities.
- Assist with document collection and coordinate with community partners.
- Maintain accurate records using Excel and other MS Office tools.
- Collaborate with eligibility teams to ensure timely enrollment.
