Job summary

Maximus is hiring Enrollment Case Managers to support the PA IEB program across Philadelphia County, with daily travel in a personal vehicle.

This role conducts in-home intake assessments, assists applicants with documenting the program, and coordinates eligibility determinations to ensure timely enrollment.

You will build relationships with community organizations, manage records in Excel, and collaborate using Word and Teams.

Qualifications

• High school diploma or GED and 4+ years of relevant professional experience or equivalent combination
• Residency in Philadelphia County, Pennsylvania
• Ability to conduct in-home visits in various home environments
• Proficiency with Microsoft Excel, Word, and Teams
• Must be able to travel daily within Philadelphia County using personal vehicle
• Ability to manage multiple tasks and meet deadlines

Responsibilities

• Conducts 3-4 in-home intake visits daily at applicants' homes
• Performs intake and facilitates eligibility determinations for waiver programs
• Presents program options and explains applicant rights and responsibilities
• Assists applicants with documentation and selecting options that fit their needs
• Develops relationships with community-based organizations to improve access and remove enrollment barriers
• Maintains accurate records and tracks applicant data in Excel; uses Word and Teams for documentation and collaboration
