Diamond Care Group seeks a reliable Certified Occupational Therapy Assistant to deliver in-home therapy under OT supervision. You will help patients improve daily living skills and safety within their home setting.

The role emphasizes hands-on patient care, documentation per Medicare guidelines, and collaboration with an interdisciplinary team. Valid Florida COTA license and CPR certification are required.

Qualifications

• Associate degree from an accredited Occupational Therapy Assistant program.
• Active Florida COTA license.
• Home health experience preferred.
• Knowledge of Medicare documentation requirements is a plus.
• Familiar with EHR such as Kinnser.
• Strong communication and time management skills.
• Possesses andmaintainscurrent CPR certification.
• Valid driver’s license and reliable transportation.

Responsabilités

• Provide occupational therapy interventions as directed by the supervising OT.
• Assist patients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting.
• Implement therapeutic exercises and activities to improve strength, coordination, and mobility.
• Educate patients and caregivers on home exercise programs and safety strategies.
• Accurately document patient visits in accordance with Medicare and home health guidelines.
• Communicate patient progress and concerns to supervising OT and interdisciplinary team.
