Job summary

Valleys seeks a Community Engagement Assistant to join a multidisciplinary team in Fairmont, WV. You will assess consumer needs, plan and implement goals, and support daily living activities including budgeting, meal prep, and transportation to appointments.

Work is performed in home and community settings, ensuring safety and regulatory compliance while promoting recovery-oriented care for individuals with complex mental health needs.

Qualifications

• High School Diploma or equivalent.
• Ability to legally transport clients.
• Must read, write and speak English.
• CPR/First Aid certification required.
• MANDT certification required.
• Adherence to Valleys policies and safety procedures.

Responsibilities

• Assess consumers needs and evaluate progress.
• Develop meal menus and shopping lists; transport groceries.
• Assist individuals in meal preparation and budgeting.
• Create appointment schedules and support daily living activities.
• Help clients follow medication plans and track progress.
• Engage clients in community-based activities and transport to appointments.
