Valley Comprehensive Community Mental Health Center seeks a Community Engagement Specialist to assess consumer needs, plan goals, and implement home and community-based support. Role includes collaboration with crisis and hospital teams to prevent involuntary hospitalization and improve daily living skills.

Position focuses on outreach, transportation, advocacy, and ensuring access to necessary services while maintaining compliance with state guidelines and organizational policies.

Qualifications

• BA/BS in Human Service Field.
• Six months to one-year experience with individuals in human services.
• Valid driver's license & ability to transport clients.
• Ability to read, write and speak the English language.
• CPR/First Aid certification.
• MANDT Certification.

Responsabilités

• Assess consumers needs and evaluate progress.
• Provide in-home skill building and transportation to appointments.
• Develop and maintain daily living skills and budgeting assistance.
• Coordinate with Service Coordinators, Hospital Liaison, and other providers.
