Job summary

AHL HealthCare Group is seeking Bridges Counselors to serve as advocates, educators, and caregivers for individuals with disabilities and mental illness in their homes and in the community. The role involves traveling around the Duluth area to meet clients in home and community settings.

Compensation ranges from $19.00 to $20.00 per hour, depending on education and experience. Part-time or full-time roles are available, with a flexible schedule and opportunities for professional growth and

Qualifications

• Age 18+ and fluent in reading, writing, and speaking English.
• Driver's license with access to a vehicle to transport clients and ability to pass MVR check.
• Ability to pass MN DHS background study.
• Mobile device capable of receiving/sending calls and using our app for documentation.

Responsibilities

• Implement person-centered, positive practice behavior support plans to promote independent living (budgeting, services, meal planning, employment).
• Meet or exceed direct client time target (90% direct, ≤10% indirect).
• Demonstrate knowledge of each consumer's history, skills, vulnerabilities, support plans, and emergency procedures.
• Provide support and supervision per support plan using therapeutic interventions while ensuring safety.

Education

Bachelor's Degree in Psychology or Social Work

Associate's Degree in a Social Services Discipline
