Job summary

AHL HealthCare Group is seeking Bridges to Success Counselors to advocate, educate, and care for individuals with disabilities and mental illness in home and community settings. You will be known as a BRIDGES Counselor within the Bridges to Success program, traveling around the Duluth area to meet clients in their homes and communities.

Qualifications

• Must be at least 18 years old and able to read, write, and speak English fluently.
• Driver's license and access to a working vehicle to transport clients.
• Must pass MN DHS background study.
• Mobile device capable of receiving/sending calls and using our app.
• Experience with individuals who have disabilities or mental illness is preferred.

Responsibilities

• Implement person-centered, positive behavior support plans to help individuals live independently.
• Meet or exceed direct client time targets with minimal indirect time.
• Understand client history, skills, vulnerabilities, support plans and emergency procedures.
• Provide support and supervision per each plan using therapeutic interventions and maintaining safety.
• Encourage engagement with local community resources within a 10-mile radius of clients' homes.
• Travel regularly within the community and transport clients as needed.
• Develop professional, respectful relationships with clients and their families.
• Maintain a clean, professional appearance as a role model for residents.

Compensation

$19.00 to $20.00 per hour, depending on education & experience

Location:

This role is responsible for traveling around the Duluth area to meet with clients in home and in the community

Hours Of Position

• Flexible scheduling!

Typical Job Duties

• Implementing and following the person-centered, positive practice behavior support plan for individuals to maintain or improve areas of independent living such as budgeting, maintaining services, meal prep/planning, employment, etc.
• Meet or exceed direct client time target of 90% with no more than 10% indirect time (driving, training, etc.)
• Demonstrate a working knowledge of consumer(s) history, skills, vulnerabilities, support plans and emergency procedures
• Provide support and supervision as defined in each support plan, using therapeutic interventions, while ensuring consumer safety
• Encourage local community setting/resources by remaining within a 10 miles radius of the clients' home
• Regularly travel in the community and transport clients
• Ability to develop and maintain professionally appropriate and respectful therapeutic relationships with each client and the client's family
• Maintain a clean, professional, and pleasant appearance for effective role modeling to the residents

Requirements For All Applicants

• You must be at least 18 years of age and be able to read, write, and speak English in a fluent manner
• Driver's license with access to working vehicle to transport clients & ability to pass MVR check
• The ability to pass a MN DHS background study
• A mobile device that can receive & send phone calls and capability of downloading our application for documentation

Additional Requirements For Counselor II Level

• A Bachelor's Degree in Psychology, Social Work, or a related field preferred
• OR an Associate's Degree in a Social Services Discipline
• OR two years (4,174 hours) of supervised experience working with individuals who exhibit challenging behaviors as well as co-occurring mental disorders or neurocognitive disorder
