Qualifications

• Experience as CNA/PCA in home care or similar setting.
• Ability to follow care plans and report changes.
• Reliable attendance and clear communication.

Responsibilities

• Personal care: bathing, grooming, dressing, hygiene, toileting.
• Ambulation, positioning, transfers, gait belt assistance.
• Meal preparation, companionship, social engagement, and light housekeeping.
• Transportation to errands and medical appointments.
• Medication reminders and prescribed ROM assistance.
• Blood glucose monitoring assistance and hospice-supportive care when trained.
