Job summary
Rocky Mountain Care Personal Care in Utah is seeking a compassionate CNA to provide one-on-one home care in the Davis-Weber area. You will assist with daily living, mobility, meals, and monitor wellbeing in patients' homes.
The role requires current Utah CNA certification, CPR/BLS, a high school diploma or equivalent, and a Food Handler's Permit, plus a valid driver’s license and reliable transportation. Mileage is reimbursed and benefits are available for full-time staff.
Qualifications

- High school diploma or equivalent.

- Valid driver's license and reliable transportation.
Responsibilities
- Assist patients with activities of daily living and personal care per care plans.
- Help patients move, reposition, and transfer safely.
- Provide mealtime assistance and support nutrition needs.
- Monitor changes in conditions, skin integrity, and overall well-being.
- Complete required documentation and communicate updates to nursing staff.
- Travel between patient homes and assisted living communities to provide scheduled care.
