Job summary

Rocky Mountain Care Personal Care is seeking a compassionate CNA to provide one-on-one care in patients' homes throughout the Davis-Weber area in Utah. This part-time role offers mileage reimbursement and the chance to build meaningful relationships by supporting patients with daily activities and comfort in familiar surroundings.

Ideal candidates are dependable, patient, and capable of working independently within a care team, with current Utah CNA and CPR/BLS certifications and a high school

Qualifications

• Current CNA certification in good standing with the state of Utah.
• Current CPR/BLS certification.
• High school diploma or equivalent.

Responsibilities

• Assist patients with activities of daily living and personal care according to individual care plans.
• Help patients safely move, reposition, and transfer as needed.
• Provide mealtime assistance and support with nutritional needs.
• Monitor and report changes in patients' conditions, skin integrity, and overall well-being to the nursing team.
• Assist with routine care needs, including weights and nutritional supplements.
• Complete required documentation and communicate important updates to nursing staff.
• Travel between patient homes and assisted living communities to provide scheduled care.
