Job summary

Rocky Mountain Care Personal Care in Utah is seeking a compassionate Certified Nursing Assistant to deliver one-on-one care in patients' homes in Woods Cross and surrounding areas.

As a full-time CNA, you will assist with daily living activities, monitor health, and report changes to the nursing team while traveling between assignments. Flexible day shifts and benefits available for full-time staff.

Qualifications

• Current CNA certification in good standing with the state of Utah.
• Current CPR/BLS certification.
• High school diploma or equivalent.
• Current Food Handler's Permit.
• Experience in long-term care, assisted living, or home care preferred.
• Reliable transportation and a valid driver's license.
• A compassionate, patient, and dependable approach to caregiving.
• Strong communication skills and the ability to work independently while being part of a care team.

Responsibilities

• Assist patients with activities of daily living and personal care per care plans.
• Help patients move, reposition, and transfer safely.
• Provide mealtime assistance and nutritional support.
• Monitor and report changes in patients' conditions, skin integrity, and well-being to the nursing team.
• Assist with routine care including weights and nutritional supplements.
• Complete required documentation and communicate updates to nursing staff.
• Travel between patient homes and assisted living communities to provide scheduled care.
