Nuclear Care Partners is hiring a CNA for part-time, in-home care in the Cortez, CO area. You will assist with ADLs, provide companionship, and support families by delivering compassionate, one-on-one care in patients' homes.

The role emphasizes reliable, mission-driven caregiving and adherence to care plans. The position offers competitive pay ($16.50–$23.00 per hour, with $1/hr night/weekend differential) and standard benefits for full-time/part-time staff, including PTO and EAP.

Qualifications

• High school diploma or GED.
• Active CNA certification (CO).
• CPR certification.
• Valid driver's license and reliable transportation.
• Ability to work independently in a home setting.
• Home health, long-term care, or hospice experience preferred.

Responsibilities

• Assist with activities of daily living (ADLs) (bathing, dressing, grooming, toileting, mobility).
• Provide companionship, emotional support, and social engagement.
• Support meal preparation, nutrition support and light housekeeping.
• Monitor and report changes in condition.
• Follow care plans and document accurately using EMR standards.
