Job summary

DCCCA is seeking a licensed social work or related professional to deliver home-based therapy and comprehensive case management across multiple Kansas sites. You will assess client needs, develop treatment plans, and provide counseling to individuals and families while coordinating with referral sources.

This role requires strong crisis response skills, excellent documentation, and the ability to engage with families, schools, and court systems.

Qualifications

• Requires MSW or related degree and relevant licensure or credentialing.
• Experience in crisis intervention and client advocacy preferred.
• Ability to coordinate care with multiple stakeholders.

Responsibilities

• Responds promptly to crisis situations and documents critical incidents.
• Provides at least 60 hours of in-home therapy per month or per contract needs.
• Facilitates initial meetings with clients and referral sources within required time frames.
• Develops treatment plans with clients and provides individual, family, and group counseling.
• Acts as client advocate and liaison with community services.
• Monitors progress using established tools and updates treatment plans accordingly.
• Maintains client records and documents contacts in the EMR.
• Completes discharge summaries and aftercare contacts as required.
• Provides progress reports to referring entities within deadlines.
• Participates in clinical supervision and collaborates with program staff.
