Job summary

Eminence Home Care is seeking compassionate Home Care Caregivers/Direct Care Workers with driving capabilities to join our team. You will provide in-home care, assist clients with transportation to medical appointments, social outings, and other activities, and work closely with families and healthcare professionals to follow tailored care plans.

Qualifications

• HS diploma or equivalent; caregiving certifications are a plus.
• Valid driver's license with a clean driving record; reliable vehicle.
• Previous home care experience preferred.
• Excellent interpersonal and communication skills.
• Physically able to assist clients with transfers and mobility.
• Demonstrated compassion, patience, and professionalism.
• Flexible availability, including evenings and weekends as needed.

Responsibilities

• Assist clients with daily living activities such as bathing, grooming, and meal preparation
• Provide companionship and emotional support to enhance clients' well-being
• Monitor and document clients' health status, reporting any changes promptly
• Administer medications as directed and comply with established care plans
• Drive clients safely to medical appointments, errands, and social activities
• Maintain client transportation logs and ensure vehicle cleanliness and safety
• Communicate effectively with clients, family members, and healthcare professionals about care needs and progress
• Perform light housekeeping to maintain a safe and clean environment
