Eminence Home Care is seeking compassionate Home Care Caregivers/Direct Care Workers with driving capabilities to join our team in Connecticut. You will provide in-home care, support daily living activities, and assist clients with transportation to medical appointments and social outings while ensuring safety and comfort.

This role emphasizes empathy, professionalism, and teamwork with clients, families, and healthcare professionals.

Qualifications

• HS diploma or equivalent required.
• Valid driver’s license with clean record.
• Reliable vehicle and ability to transport clients.
• Previous home care experience preferred.
• Excellent communication and interpersonal skills.
• Flexible availability including evenings and weekends.
• Ability to assist with transfers and mobility.

Responsibilities

• Assist clients with daily living activities such as bathing, grooming, and meal prep.
• Provide companionship and emotional support.
• Monitor health status and report changes promptly.
• Administer medications as directed per care plans.
• Drive clients to medical appointments, errands, and social activities.
• Maintain transportation logs and vehicle safety.
• Communicate with clients, families, and healthcare professionals about needs and progress.
• Perform light housekeeping to maintain a safe environment.
