Job summary

At Home Reach seeks a Direct Support Professional to provide in-home care for individuals with disabilities in Idaho Falls. You will support daily living skills, promote independence, and maintain a safe, respectful living environment within the home.

Responsibilities include assisting with activities, transportation to appointments, budgeting help, and community participation. You will document progress, communicate with the team, and participate in ongoing training to ensure quality services.

Qualifications

• Must be 18+ years old with a high school diploma or GED.
• Ability to communicate professionally with participants, families, staff, and community members.
• Ability to read and write in English and follow written/verbal instructions.

Responsibilities

• Assist with activities of daily living and independent living skills.
• Support daytime, recreational, vocational, and community activities.
• Maintain hygiene, healthy habits, and safe living environments.
• Complete required documentation and shift communications.
• Report concerns to QIDP/Administrator and supervisory staff.
