Job summary

At Home Reach in Pocatello, ID is seeking a Direct Support Professional for weekdays to provide in-home care for individuals with developmental disabilities, helping with daily living skills and community participation.

Requirements include 18+, HS diploma or GED, English proficiency, a valid driver’s license, and ability to lift up to 50 pounds. CPR/First Aid certification is preferred; shifts include AM, PM, and Noc.

Qualifications

• Must be at least eighteen (18) years of age.
• High school diploma or GED.
• Ability to communicate effectively and professionally.
• Ability to read, write, and communicate effectively in English.
• Ability to follow written and verbal instructions.
• Ability to work independently and as part of a team.
• Ability to safely lift, transfer, and assist participants up to 50 pounds.
• Active driver’s license and reliable transportation.

Responsibilities

• Assist participants with activities of daily living and independent living skills.
• Support daytime, recreational, vocational, and community activities.
• Encourage and promote good personal hygiene, healthy habits, and appropriate behavior.
• Be Medication Certified and able to pass medications.
• Assist participants with budgeting, groceries, meals, and daily chores.
• Complete required documentation including daily logs and shift notes.
• Communicate participant concerns, behavioral changes, incidents, and updates to supervisors.
• Provide transportation to appointments, activities, and community outings as needed.
