Job summary

PACE SEMI is seeking a Direct Home Care Worker to provide compassionate in-home and community support, including transport and day health center assistance. You will follow individual care plans and assist with daily activities, meals, and basic health tasks.

The role requires 2 years of relevant experience, high school diploma or GED, and formal Direct Care training. You must maintain documentation, complete in-service training, and have a valid driver's license for local travel.

Qualifications

• Minimum High School Diploma or GED.
• 2 years of relevant work experience.
• Formal training program in Direct Care, Nurse Assistant, Medical Assistant, or equivalent.
• Annual in-service training including infection control, TB testing, safety training, and BLS.
• Ability to establish interpersonal relationships and respect participants and co-workers.
• Must be medically cleared for communicable diseases and up-to-date on immunizations.
• Must have reliable transportation and a valid driver's license; driving within catchment area required.

Responsibilities

• Provide personal care and assistance with activities of daily living.
• Assist with meals, toileting/incontinence, and simple treatments.
• Assist with shopping, laundry, homemaking, and transportation to appointments.
• Provide medication reminders and maintain accurate documentation for each visit.
• Respond to emergencies per policies and procedures; operate within scope of practice.
