Job summary

• Strong assessment, clinical, and communication skills.

Responsibilities

• Deliver person-centered nursing care in clients' homes, including comprehensive assessments, care plan development, and updates.
• Administer medications and treatments while monitoring both chronic and acute conditions.
• Coordinate with physicians, behavioral health specialists, therapists, and social workers to ensure evidence-based, outcomes-focused care.
• Educate patients and families on health management and safety practices.
• Accurately document all care activities in the EMR system.
• Promote safety, infection control, and support quality improvement initiatives.
• Collaborate with public-sector partners to ensure integrated care delivery.
