Job summary

SHHC is seeking a Visiting Nurse - RN to deliver compassionate in-home care in the Boston area. The role includes conducting comprehensive home assessments, developing care plans, administering treatments, and coordinating with a multidisciplinary team to promote recovery and independence for patients.

Responsibilities include overseeing patient progress, ensuring safety, and providing education to patients and families. This is a part-time role with flexible scheduling in a community setting.

Qualifications

• Current RN license in the state of practice.
• Nursing degree (Associate's or Bachelor's).
• Minimum of one year clinical nursing experience, home health preferred.
• BLS certification from American Heart Association or equivalent.

Responsibilities

• Conduct home health assessments to evaluate physical, emotional, and social health needs.
• Develop and update individualized nursing care plans.
• Administer medications and therapies per physician orders; document outcomes.
• Coordinate with physicians, therapists, social workers and families for continuity of care.
• Maintain accurate clinical documentation in compliance with standards and policies.
• Identify changes in patient conditions and communicate with providers.
