Job summary

Well Care Health in North Carolina is seeking a compassionate Home Health Registered Nurse to join our growing team and provide high-quality in-home patient care. You will assess patients, create and implement care plans, educate families, and coordinate with a multidisciplinary team to ensure optimal outcomes.

This role requires a valid NC nursing license and CPR certification, with a preference for 1 year RN experience and 2+ years total clinical experience.

Qualifications

• Education: Graduate of an accredited school of nursing (AD, Diploma or BSN).
• Licensure: Current RN license in NC/SC; CPR certification required.
• Experience: 1 year RN; total 2+ years clinical experience; home health preferred.
• Physical/medical: capable of lifting and assisting patients, driving to visits.

Responsibilities

• Assess and monitor patients in their home environment.
• Develop and implement individualized care plans.
• Educate patients and families on health management and self-care.
