Job summary

Woven Home Care seeks a Care Manager to serve clients and caregivers across St. Louis & St. Charles Counties, Missouri. You will conduct in-home assessments, build care plans that guide each shift, coach caregivers, and stay close to clients and families so problems get solved before they become problems.

Qualifications

• Bachelor's degree preferred and licensure where applicable.
• Strong communication, coaching, and documentation skills.
• Comfortable with technology — care plans, schedules, and notes live in our platform.

Responsibilities

• Conduct in-home client assessments and reassessments.
• Develop and maintain individualized care plans.
• Ensure documentation, compliance, and safety standards.
• Coach and supervise caregivers.
• Coordinate with scheduling staff for coverage.
• Represent Woven in the community and build referrals.
