Continuum - St. Louis Home Care in the St. Louis region seeks an experienced Care Manager to guide families through intake, assessments, and coordinated care navigation. This role focuses on compassionate support, thorough documentation, and collaboration with care teams.

The ideal candidate will have MSW (or BSW) and strong communication, time management, and organizational skills, with reliable transportation for home visits and community referrals.

Qualifications

Responsibilities

• Conduct intake assessments with prospective clients and family members.

• Conduct new caregiver introductions.

• Participate in an on-call rotation to provide after-hours support and guidance to clients, families, and caregivers as needed.
• Build trusting relationships with clients and families throughout the care journey.

We are seeking an experienced and compassionate Care Manager to join our team and serve as a vital resource for new and existing clients. This position is ideal for a social worker who enjoys connecting with families, conducting assessments, coordinating services, and helping clients successfully navigate the continuum of care.

Care Manager Overview

The Care Manager serves as the first point of contact for prospective clients and their families. This role is responsible for conducting intake assessments, completing in-home consultations, gathering client information, developing care recommendations, and assisting clients and families with care navigation and community resources. Success in this role requires exceptional time management, organization, communication, and follow-through. The ideal candidate is comfortable managing multiple referrals, prioritizing tasks, and maintaining accurate documentation while delivering a high level of customer service.
