Job summary

FreedomCare in Missouri is seeking a Care Manager, RN to perform initial in-home assessments, coordinate care, and educate patients and caregivers while ensuring quality and regulatory compliance.

This field-based role requires frequent travel to patient homes in Springfield, MO. You will collaborate with interdisciplinary teams to develop care plans, monitor outcomes, and support authorized services.

Qualifications

• Diploma from an accredited nursing program.
• Valid and active Registered Nurse (RN) license in the State of Missouri.
• Valid driver’s license and car insurance required, with means of reliable transportation.

Responsibilities

• Conduct initial in-person clinical assessments and coordinate timely service activation.
• Develop and manage individualized plans of care, documenting revisions as needs change.
• Educate patients, caregivers, and internal teams regarding care needs, safety considerations, and service requirements.
• Coordinate patient care through timely communication with physicians, clinical disciplines, and community resources.
• Provide ongoing education and coaching to support best practices and caregiver competency.
• Review caregiver documentation and respond to changes in patient condition or service needs.
• Identify changes in condition or safety concerns and escalate per clinical protocols.
• Complete and document assessments, visits, observations, care-plan updates, incidents, and follow-ups.
• Support authorization, renewal, start-of-care, and continued-service processes.
• Maintain compliance with regulatory requirements and company policies.
