Job summary

Phoenix Home Care and Hospice in Missouri is seeking a highly organized Care Coordinator to ensure our clients receive exceptional care. You will serve as the connection between clients, families, caregivers, and nurses, coordinating daily schedules and resolving issues.

This role requires strong communication, multitasking, and problem-solving skills. You will partner with clinical and operations teams, maintain documentation, and support field staff to uphold quality care and a compassionate,

Qualifications

• High School Diploma or equivalent.
• Previous office, scheduling, staffing, or healthcare experience preferred.
• Experience working with elderly, disabled, or medically fragile individuals preferred.
• Strong communication, customer service, and problem-solving skills.
• Ability to multitask and prioritize in a fast-moving environment.
• Professional, positive, and team-oriented attitude.
• Strong organizational and computer skills.

Responsibilities

• Coordinate daily schedules for caregivers and nursing staff
• Build and maintain strong relationships with clients and families
• Communicate schedule changes and care updates promptly
• Support and assist field staff with day-to-day needs
• Investigate and resolve client and employee concerns
• Maintain accurate documentation and reporting
