Innovative Integrated Health in Bakersfield, CA, is seeking a dedicated CNA to deliver compassionate personal care and nursing support under the Center Manager. You will work within an interdisciplinary team to assist participants with daily living activities while documenting care and ensuring safety.

Qualifications include a CA CNA license, GED or higher, CPR/First Aid, and a patient-centered approach. The role requires flexibility, strong communication, and adherence to center policies.

Responsibilities

• Participate in interdisciplinary team assessments and monitor participant condition.
• Provide personal care for activities of daily living (toileting, bathing, eating, grooming).
• Provide nursing care consistent with CNA certification and center policies.
• Ensure participant safety and respond to emergencies.
• Offer behavioral support and remind participants about care plans.
• Assist with daily activities and light housekeeping.
• Identify signs of health or behavioral changes and report to supervisor.
• Maintain written records and timely reports; protect confidentiality.
• Attend staff meetings and participate in care planning.
• Support adherence to center policies and flexible scheduling.
• Possibly use a personal vehicle for transportation as needed.
