Job summary

Innovative Integrated Health Inc. seeks a dedicated CNA to support senior participants in a center-based setting. You will perform personal care tasks, monitor health status, and collaborate with an interdisciplinary team to maintain independence and safety.

Requirements include an active California CNA license, CPR/First Aid, GED or high school diploma, and a minimum of one year with frail elderly populations. A food handler’s license is preferred; re-certification may be required.

Qualifications

• GED or high school diploma required.
• Ability to document and report changes in participants' condition.
• Strong communication with participants, families, and staff.

Responsibilities

• Provide personal care assistance to maintain participant independence in activities of daily living.
• Assist with mobility, bathing, incontinence, grooming, meals, and activities of daily living.
• Report adverse changes and participate in care planning with the interdisciplinary team.
• Escort participants to appointments and maintain written records of services provided.
