Job summary

InnovAge is seeking an In Home Services CNA to provide routine care to participants in their homes or InnovAge facilities. You will follow care plans, monitor vital signs, assist with ADLs, document care, and collaborate with RNs and the care team to support independence.

The position requires a current CNA license, First Aid and BLS, a valid driver's license, reliable transportation, and at least one year of experience with the frail or elderly. HS Diploma or GED is acceptable.

Qualifications

• Current and valid state CNA license required.
• First Aid and BLS certifications required prior to hire.
• Driver’s license and reliable transportation required.
• Minimum one year experience with frail or elderly.
• HS Diploma or GED acceptable in lieu of education.

Responsibilities

• Provide routine care and assist with ADLs and daily living activities.
• Document care provided in accordance with policies and regulations.
• Monitor health status and vital signs; report to RN.
• Collaborate with multidisciplinary teams in care planning.
• Assist with meals, positioning, transfers, and comfort.
• Complete visit records timely; attend required in-services.
