Responsibilities

Serve as main liaison for clients and families, ensuring satisfaction with care services.
Carry out 7, 30, 60, 90 days visits of clients.
Introduce clients to new caregivers (Meet and Greet).
Conduct initial intake calls and assist with onboarding new clients.
Maintain ongoing communication with clients to monitor care needs and adjust as needed.
Address and resolve client concerns promptly and professionally.
Develop individualized care plans and update them as conditions change.
Monitor client progress and ensure care plans are followed.
