Job summary

Team Select Home Care of Colorado, LLC in Loveland, CO is seeking a compassionate Certified Nursing Assistant (CNA) to provide skilled personal care and ADLs in patients' homes under the supervision of an RN. You will follow the care plan, assist with daily activities, and report changes to the RN promptly.

Qualifications

Responsibilities

• Provide direct patient and ADL care as ordered in plan of care.
• Assist with household tasks, meals, and delegated duties.
• Support simple procedures as delegated by RN or therapy.
• Assist with ambulation/exercises as assigned.
• Help with medications ordinarily self-administered as allowed by state law.
• Maintain infection control practices and report changes to RN.
• Complete visit records timely and accurately.
• Attend meetings/conferences as scheduled.
• Follow agency policies and document care performed.
