Job summary

Aging in Place LLC is seeking a dedicated CNA/Home Health Aide to join our in-home care team in Ohio. You will assist patients with ADLs, provide personal care, monitor well-being, and support family members in daily routines in the patient’s home environment.

Responsibilities include helping with bathing, dressing, grooming, eating, mobility, light housekeeping, medication reminders, and documenting daily activities. Must follow safety protocols and maintain compassionate, patient-centered care.

Qualifications

• Must be able to read and interpret documents.
• Able to communicate effectively with patients and families.
• Able to write care notes documenting activities.

Responsibilities

• Assist with patient ADLs including bathing, dressing, grooming, eating, and incontinence care.
• Provide companionship and engage patient in activities.
• Accompany patient to appointments and on errands.
• Inform supervisor of any changes in condition or needs.
• Perform light housework to maintain a clean home environment.
• Remind patient to take medications; do not administer medications.
• Document activities and findings after each visit.
