Job summary

A Place at Home is seeking a Home Health Aide to join our in-home care team in Ohio. You will provide compassionate assistance with daily living activities under RN supervision, helping clients stay independent in their own homes.

Key tasks include ADLs, meal support, medication reminders, light housekeeping, mobility assistance, and documenting changes. Candidates must have HHA certification, pass background check, and possess a valid driver's license.

Qualifications

• Must have state-approved Home Health Aide certification.
• Pass a background check and drug test.
• Strong communication skills, written and verbal.
• Compassionate and empathetic with a desire to help others.
• Ability to work independently with patience and understanding.

Responsibilities

• Assist with ADLs such as bathing, dressing, grooming, and toileting.
• Assist with meal planning and preparation, observing dietary restrictions.
• Provide medication reminders per care plan.
• Perform light housekeeping to maintain a clean home environment.
• Assist with mobility and transfers ensuring client safety.
• Provide companionship and build trust with clients.
• Document and report changes in client condition to supervising nurse.
• Maintain HIPAA privacy and protect client PHI.
• Participate in ongoing training and professional development.
