Job summary

Angels Care Home Health in Oklahoma is seeking a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy to homebound patients and coordinate care with the interdisciplinary team. You will conduct in-home assessments, develop care plans, deliver behavioral health nursing interventions, and ensure accurate documentation in EMR while meeting regulatory standards.

The role requires current RN license and 1–2 years in psychiatric settings.

Qualifications

Current state RN license.
1–2 years nursing experience in psychiatric settings.
Reliable transportation with valid auto liability insurance.
Current CPR certification and Hepatitis consent/declination.

Responsibilities

Conduct comprehensive in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
Develop, implement, and regularly update individualized plans of care to ensure quality and continuity of services.
Provide skilled nursing interventions related to behavioral health and applicable medical conditions within scope of practice.
Coordinate care with physicians, interdisciplinary team members, patients, families, and referral agencies.
Ensure patient medication profiles are accurate and current; complete admissions and discharges per regulatory standards.
