Job summary

Angels Care Home Health seeks a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy for homebound patients. You will coordinate care with the interdisciplinary team, patients, families, and referring agencies, ensuring medical necessity and homebound status are documented.

You must have an RN license, 1–2 years in psychiatric/home health settings, CPR, a valid driver’s license, and reliable transportation.

Qualifications

• Must have 1–2 years of nursing experience in an acute treatment unit in psychiatric settings or psychiatric home care.
• Current state RN license.
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with valid auto insurance and a valid driver’s license.

Responsibilities

• Conduct in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
• Develop, implement, and update individualized plans of care to ensure quality and continuity of services.
• Provide skilled nursing interventions related to behavioral health and applicable medical conditions.
• Coordinate care with physicians, interdisciplinary team, patients, families, and referral agencies.
• Ensure patient medication profiles are accurate and current at all times.
• Complete admissions, resumptions of care, recertifications, and discharges per standards.
• Demonstrate proficient use of EMR systems and timely documentation.
