Job summary

ConcertoCare is seeking an Advanced Practice Provider (NP or PA) to deliver in-home clinical services to patients with complex needs in Washington state. You will work within a multidisciplinary team to improve outcomes and quality of life, including care coordination and telehealth options.

The role emphasizes top-of-license practice, patient-centered care, and ongoing education through expert teams in geriatric medicine, palliative care, and care management.

Qualifications

• Licensed NP (WA) or Board-Certified PA with active licensure.
• Experience delivering in-home care to adults with complex needs.
• Telehealth experience preferred.
• Active DEA licensure and state CS registration, as applicable.

Responsibilities

• Deliver in-home and virtual care with a patient-centered approach.
• Collaborate with nurses, social workers, case managers, and pharmacists.
• Lead complex medication management and care coordination.
• Document and code accurately for care activities.
• Participate in team huddles and clinical meetings.
• Support transitions of care and address social determinants of health.
