Job summary

ConcertoCare in New York is seeking an Advanced Practice Provider (NP or PA) to deliver in-home clinical services to patients with complex needs. You will collaborate with a multidisciplinary team to improve outcomes and support families in care management.

Qualifications

• Licensed NP with ANCC/AANP certification or Board-Certified PA with active licensure.
• Experience delivering in-home and telehealth care to complex patients.
• DEA license and state controlled substance registration where applicable.

Responsibilities

• Deliver in-home health assessments and manage acute and chronic conditions.
• Collaborate with nurses, social workers, case managers, and pharmacists.
• Lead complex med management and address social determinants of health.
• Foster patient engagement and maintain timely documentation and coding.
• Participate in after-hours on-call and team cross-coverage as needed.
