Monogram Health is seeking compassionate Advanced Practice Providers (NPs or PAs) to deliver high-quality in-home care to patients with multiple chronic conditions. The role works within an integrated multidisciplinary care team and includes telehealth, in-home visits, and a value-based care model.

Responsibilities include assessments, patient education, care coordination, documentation, and participation in virtual team meetings.

Qualifikationen

• Active and unrestricted RN/NP or PA license to practice.
• Board certification (NP: ANCC/AANP; PA: NCCPA).
• Current DEA certificate or eligibility.
• Minimum of two years of direct patient care.
• Field-based role requiring travel to patient homes.
• Experience with EHR systems (Athena, Salesforce, Epic).
• Experience with annual wellness or comprehensive visits preferred.
• Background in managed care, population health, or value-based care preferred.

Aufgaben

• Provide evidence-based, patient-focused care in patient homes.
• Perform comprehensive assessments and wellness visits.
• Educate patients and caregivers on disease management and medications.
• Participate in virtual integrated care team meetings.
• Engage in quality assurance and documentation reviews for compliance.
• May practice in SNFs, ALFs, hospitals, and other settings.
• Coordinate care with PCPs, specialists, and ancillary services.
• Complete documentation in a timely manner.
