Job summary

Cooper Norcross Health is seeking an Advanced Practice Provider (NP/PA) to conduct in-home visits across Burlington, Camden and Gloucester counties. The APP will assess patients, develop care plans, and coordinate treatment with supervising physicians to prevent readmissions and improve health outcomes.

The role requires at least 3 years of adult/geriatric APP experience, a master's degree, and knowledge of Primary Care and Chronic Care Management.

Qualifications

• Minimum of 3 years working with the adult and geriatric population as an APP (Nurse Practitioner or Physician Assistant).
• Current knowledge and understanding of Primary Care, Chronic Care Management, and Best Practices.
• Home based Primary Care experience preferred.

Responsibilities

• Perform patient history and physical examination to determine a working differential diagnosis and develop an individualized plan of care.
• Order, perform, supervise, and interpret diagnostic tests and prescribe pharmacologic and non-pharmacologic therapies.
• Provide teaching, education and counseling to individuals, families and groups.
• Refer to a specialist or participate in dual management with supervising physician for uncommon or unstable conditions.
• Emphasize health promotion and disease prevention; act as health care researcher, educator and patient advocate.
• Prescribe medications as clinically indicated and document in the patient chart.
• Maintain continuing education per certifying requirements and ensure up-to-date clinical skills.
