Job summary

Cooper Norcross Health is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for in-home post-discharge management of at-risk adults. Qualifications

• Minimum of 3 years working with adult and geriatric populations as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant).
• Master's degree required - Nurse Practitioner or Physician Assistant.
• NJ-APN & NJ-RN or NJ-PA License required.
• American Heart Association BLS certification required.
• Valid State Driver's License with a clean DMV record.

Responsibilities

• Perform a patient history and physical examination to determine a working differential diagnosis and develop an individualized plan of care.
• Order, perform, supervise, and interpret diagnostic and laboratory tests, and prescribe pharmacologic and non-pharmacologic therapies.
• Provide teaching, education and counseling to individuals, families, and groups.
• Diagnose and manage acute and chronic illnesses and emphasize health promotion and disease prevention.
• Collaborate with supervising/collaborating physician and other health care professionals to manage patient health needs.
