Job summary

Cooper Norcross Health seeks a per diem Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to provide in-home care for adults and the elderly in New Jersey. The APP will perform history taking, exams, manage diagnostics, and coordinate with supervising physicians during home visits.

Responsibilities include prescribing medications when indicated, patient education, and health promotion. Eligible candidates hold an AHA BLS certificate and NJ APN or PA licensure with at least 3

Qualifications

• Minimum of 3 years working with adult and geriatric populations as an Advanced Practice Provider.
• MASTERS is required - Nurse Practitioner or Physician Assistant.
• American Heart Association BLS certification is required

Responsibilities

• Take patient history and perform physical examinations to inform care plans.
• Order, perform, supervise, and interpret diagnostic tests; manage pharmacologic and non-pharmacologic therapies.
• Prescribe medications as appropriate and document prescriptions in the chart.
• Provide education and counseling to patients and families; promote health and disease prevention.
• Collaborate with supervising or collaborating physicians to manage patient care and referrals.
