Job summary

Monogram Health is seeking an Advanced Practice Provider to join its integrated Care Team. The role involves delivering personalized medical care primarily for patients with chronic conditions, including CKD and ESRD. Responsibilities include conducting wellness exams, documenting patient histories, and coordinating care with healthcare providers.

Qualifications

• Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
• Must be proficient with medical instruments required by the work.
• Sound understanding of all federal and state regulations including HIPAA and OSHA.

Responsibilities

• Conduct comprehensive annual wellness exams on patients in their homes.
• Educate patients about benefits and programs to improve their health.
• Coordinate care with primary care providers and specialists.

Skills

Active and unrestricted Registered Nurse/Nurse Practitioner or Physician Assistant license

Communication skills
